I, as parent/guardian authorize any first aid or emergency medical care that become necessary for my child while he or she is participating in the Plantation Palms Junior Golf Camp. I understand that I will be contacted immediately at the emergency contact number listed above. In consideration of my acceptance of my child’s entry into the program, I hereby, for myself and my child, our heirs, executors, administrators and personal representative, discharge, waive and release Ace Golf, Plantation Palms Golf Club, its agents and employees from responsibility for injury or death arising from participation in the Plantation Palms Junior Golf Camp. 

By executing this release I assure, on behalf of my child, all risk and injury or loss to which he or she may be exposed.
